Former Prairie Family Medicine Patients

At the end of 2022, the family practice component of Prairie Family Medicine was acquired by Praxis Health.  The Mental Health department along with Migraine & Neurospasticity Intervention services formed Hemisphere Health, pllc.  Continuing to see your regular provider for these services should be a seamless transition.  Our new location is at 909 W Canfield Ave. in Coeur d'Alene in the Marblewood Center.
